In 2000 I tore some of the cartilage connecting my sternum to my ribs catastrophically. It sounded like ripping a head of lettuce and wasn't fully healed for about 18 months. For a long time that was my 10 on the subjective pain scale.

Last night I waited about 6 hours in a Portland ER to get IV antibiotics for an infected abscess on my thigh. It was about the size of a buried golf ball and I couldn't walk well. Regular antibiotics during the week hadn't helped much.

They did an ultrasound and found there was a core of pus and told me they had to drain it. They gave me morphine for the pain.

It wasn't enough. They were going to inject Lidocaine directly into the abscess.

The needle going in was... unpleasant because the abscess was so swollen and tender. It was about as bad as a tattoo needle drawing a line. But when the doctor injected the actual Lidocaine into it it felt like boiling oil. Absolutely the worst pain I've ever felt. Pain that blotted out the world. She did that four times.

Yes, I passed out.

I see this often. ED is fine for plenty and this is bread and butter, but they aren't too sophisticated at really improving their technique.

Your abscess is already under pressure and they add pressure to it. Lido hurts anyways because its unbuffered in the ED setting. So they just top off the swollen mass of tense skin.

The abscess needs pressure relief, but also a reasonably large incision (for the abscess size) to prevent recurrence, and also if deep probably some level of blunt manipulation inside for loculations.

This can all be made easier by numbing the surface, which sucks little anyways, and using a #11 blade to poke the abscess releasing pressure. Then add the extra lidocaine in areas need for the specific abscess. Then extend the incision to the appropriate size.

That's not needed. You're just trying to get a bigger yacht than me. It's a cutaneous abscess in a non dangerous, non-face place. Derm takes too long to get seen and this can be done by just about anyone that knows how used local anesthetic.

My GF always buffers. It brings them back to the yard. I would prefer to not have anything to do with boat ownership but happy to ride on your yacht.

I've never had a kidney stone, but I passed a couple of gall stones a week for 5 months in the beginning of 2004. I agree about wanting to pass out. It felt like a 4-hour heart attack.

Splinty @Splinty Just asked sugar momma. She does ring blocks. Says it takes 10 minutes longer but greatly reduces pain. She just did one this week. That’s the difference between a derm and an ER.

Kidney stones twice, cauliflower ear popped, torn intercostal muscles (wrestling defending gut wrench, cracked ribs (stomped out by Portuguese low life’s), broken nose (twice once wrestling, once car wreck) fixed manually, pneumothorax (torn lung).
Kidney stones was the worst pain for me, both times I ended up on all fours on the hospital floor puking.
Pneumothorax was horrible pain and scary because I didn’t know what was going on. You get a small tear on your lung and besides the pain the air released into your core and amplifies all the sound. I was lying in the couch in crazy pain and could hear my heart beating, not really the best but that weird squish sound the contractions make. It scared the hell out of me.
Recovery after penis reduction surgery was brutal too.

Sounds horrible. Hopefully your recovery is easy.

My worst was a drilling at the dentist. Fucker didn't wait long enough for me to go numb or he simply drilled too deep - but he hit raw nerve. Screaming pain that shot through my brain. I swear I thought my head was going to explode.

He said something like "That nerve is dead, you shouldn't feel anything". Fuck you it's dead. That fucking sucked. He ended up sending me to a specialist for a root canal, which sounds bad but was actually a walk in the park.

The other "10" pain for me was a back spasm that left me on my hands and knees for the better part of an hour. Everytime I tried to move there was a paralyzing pain so bad that I couldn't breathe and damned near passed out. It eventually subsided, but every muscle in my body hurt for a week - a result of all my muscles tensing up from the spasms. I wouldn't wish that shit on anyone.

Back spasms are no joke! I always thought that people in pro sports that had to miss events with them were pansies until I started having them. I thought I had a kidney stone that was shooting through my appendix and out my back I was in so much pain.

A couple of hands full of flexoril and a week later I was good to go again... Until the next time.




Not quite as much pain but more constant and seemingly impossible to escape was when I had worked and coughed myself into costochondritis. Breathe in and it hurts. Exhale and it hurts. Swallow something hot and it hurts. Lay flat and it hurts. Bend over and it hurts. Take a shit and it hurts. It was worse than when I broke my ribs. At least I was able to find a spot that wasn't complete misery then.

No doubt. Derm def subject matter experts and always have incentive for better customer service with being outpatient and all that private pay . It's just derm overkill for I&D if the ED would take 5 minutes to consider technique. They can do it. They just don't have the incentive as their pay and training is all comers volume based.
