From reading everything his description on the YouTube video of relating this closer to altitude sickness is kind of accurate.
In acute altitude sickness your lungs are working fine. The oxygen level changes cause your blood vessels to increase their resistance at the lungs.
This all leads to lungs that are moving air really well but the air is hitting blood vessels that are tight and not flowing well.

This is what they are saying is happening with covid 19 for most of these patients earlier in the course. This is directly opposite of ARDS in which the lungs get inflammatory process that is well known to commence in several distinct phases. In that case the lungs are stiffer, inflamed, and not moving air across to blood vessels that are presumably working perfectly fine ready to take the air if the lungs would just start working.

Since I am not an intensivist, this could be a real challenge and differentiating which patients are typical covid19 blood vessel shunting with good lungs and which ones have gone into ARDS which is absolutely a possibility for any systemic wide infection like this regardless of usual disease pattern.

Ignore the fine print: "However, the recent rise in the number of locally transmitted cases – those with no link to someone who was infected overseas – is a worry for authorities. At 75, there are more than triple the number of community transmission cases than there were a week ago, and Dr Sutton said these cases would grow as testing expanded to public-facing workforces including healthcare workers, police and prison staff".

See? The problem is testing. Too much testing. Our state has been on the forefront of not testing anybody, testing half as much as any other state in Australia and refusing to test for community transmission. So we nailed it! If you don't test anybody, then you too can flatten the curve in no time. Hopefully this news story means that all of my fellow Victorians stop worrying about social distancing now, as all is well.

This guy was one of my attendings. He's probably one of the smartest people I've ever met in my entire life. You know how some people become an emergency doctor or some people becoming an internal medicine doctor? So he decided to do both. No biggie. Still works doing both.

He bought into what that New York City doctor is saying.

He's suggesting that the solution is high flow nasal cannula (lots of oxygen up front) and then only intubate when you have to. Still very subjective on when choosing to intubate. But all the nerds and the emergency medicine critical care world seem to be going this route and away from the ARDS route.
If you were going to explain this to a retarded 5 year old how would you do that?

My brother is going to start making mask for the family,based off the business insider article posted earlier. They say you can wash the blue towel masks 3 times. Do you think steam sterilizing in one of those baby bottle sterilizers would work?

A dehydrator would work.
A simple toaster oven on warm or regular oven.
165 °for 30 minutes would kill SARS, so double it to sleep better.

Why do you think steam sterilization would not work? My scientist wife seems to think it should work if you run it for say 6 minutes + instead of the 4 you do for bottles.

COVID-19 & other cv's are humidity thrivers, that's my understanding.
Hot and dry is what "kills" it.

I know you’re joking but for the benefit of the others who didn’t get it... @Splinty can shoot me later for butchering this.

My understanding is there could be two main issues causing major lack of oxygen issues:

Lungs are working fine but the blood vessels are not and they’re UN able to get the oxygen needed.

or

Blood vessels are working fine but the lungs aren’t able to bring oxygen that is needed. (This seems to be the issue that most folks think is going on but could not be)

There’s a difference in approach to either problem but knowing which issues is affecting the particular patient at hand is still a huge problem and not easy to determine and proper treatment isn’t always what is administered. IE if the lungs are working fine but the blood vessels are not, intubation may not be the best solution in the beginning which seems to be what most places are doing sooner than later.

Now go back to playing games you kids.
