The term transgendered succinctly describes a person of a certain biological sex living life as another gender role. It intrinsically describes a situation in which there is not a biological sex change.

I propose psychiatric help. Rather than people pretending a man in a dress is a woman. Or vice versa. I believe it's a mental disorder that with the correct amount of medical study could in future be addressed. Rather than encourage the mental illness we should look to help.

This is true for only refractory cases. It remains somewhat controversial but less so when other treatments have failed.
Treatment begins with psychotherapy. Simple therapy and being allowed to live as a man/woman is enough for many.
Hormones are used for some and surgery for even fewer.
All transgendered treatment begins with that. This is limited in efficacy if they are home in an environment that rejects them for living as a different gender.
In places that have long standing "third genders" you see significantly less suicide, suggesting that a portion of the problem is just everyone giving a fuck so much what someone is doing with their own body and dress.

You're confusing the two terms.
They can't biologically change sex. Gender doesn't require a physical change. But can be done for aesthetic reasons. Therapy is very much involved in the equation.
