Every American war. 1 out of 3 for blue on blue is not a good statistic.

It's well known over here that the American military are more trigger happy and more likely to commit "friendly fire".
I agree with Leigh.
I remember reading reports during the first gulf war of British troops suffering casualties from US fire.
Canadian paratroopers died because of a trigger happy a-10 pilot in Afghanistan.
I read a book written by a Canadian vet in Afghanistan. There's countless anecdotes of very close-calls friendly-fire incidents with... You guess it. U.S. army.

A-10 pilots are the worst. We called them marine killers.
An A-10 pilot also killed 9 British troops in the Gulf War when he destroyed 2 Warrior IFVs. It was something like 20% of British casualties.

The US also killed 35 of their own troops, contributing to a similar percentage of casualties.
