I have a lot of patients that take their blood pressure medication for years. Then they stop because their blood pressure has been so well controlled, why do they need the meds?!??!??

They don't have a heart attack or stroke... For a while.

Depends on the cause. Much of its genetic.
But if, for instance, you are obese and become not obese there's a chance you can come off the meds.
