I don't know if I've ever met an admin that cares more about the patient than the dollar.

We've removed obstetric services and and pediatric services out of nearly every community hospital. Neither pay very much and even adult medicine when it comes to hospitalist typically runs at a loss.
All of this is then covered by ancillaries and surgical services.
I'm not sure if this all causes the grotesque abuse of the patient's wallet or if the same hospitals that lobby for certain reimbursement to be more than others do this by design.
I was mostly just making a throw away joke, but I also think we just stumbled across why patient capacity is still lacking during this 4th wave of covid.

Well of course you do.

I'm not.

A labor deck of vaccinated nurses will be less likely at any given time to have an active infection going on.
In the event that a breakthrough infection occurs said infection will be transmissible for a shorter period of time.
So fire all the ones that object?

Do you realize the sheer number of experienced and qualified healthcare professionals you are talking about?

Doesn't make a lick of sense sir.

If you feel sick stay home...problem solved.

Frequently testing the unvaccinated and not the vaccinated? What bizarro science world are we in?

What are you basing this statement on? I thought that with Delta vaccination didn't make a difference.

I'll pull the study again in a little bit. Voice typing in trying to eat while I run back to the hospital on my post call day.

Peak viral loads are the same but decrease faster in the vaccinated.

I should have said likely transmissible for a shorter period of time but since we've been using both viral load on the original strain as well as viral load peaking to justify increase transmissibility of the delta it seems the continued common standard.

No difference in the hepatitis B or flu shots or TB tests that they already take.
Zero sympathy. There's nothing new here other than politics.

I'm not worried about the studies, I'm happy to take your word for it, I was just looking for clarification as I've heard many different stories about spread by vaccinated people. Last I had heard from Dr Truth Science was that vaccination didn't impact spread when it came to Delta.

OK
I strongly disagree that it is nothing new

I wonder if he actually used those words. But you would have to decide what That phrase actually means. Vaccination absolutely impacts spread when it comes to Delta. Those with breakthrough infections initially have similar viral load and ability to spread.

A quick synopsis of some studies feel free to look at the main sources.

And one thing that all studies have been pretty consistent is that if you're vaccinated you're less likely to get infected in the first place. You can't get someone else infected if you don't get infected first.

So once again, a vaccinated labor deck will have less chance of someone working there having covid to spread than an unvaccinated labor deck.

I can't ignore this part, doc:

Data from Provincetown, Massachusetts, suggest similar findings. An August report from the US Centers for Disease Control and Prevention (CDC) showed that following large gatherings in the beach town, nearly three-quarters of 469 new COVID-19 cases that occurred in the state were in vaccinated people2. Both vaccinated and unvaccinated people had comparably low Ct values, indicating high viral loads, and of the 133 samples sequenced, 90% were identified as Delta. The findings prompted the CDC to update its guidance on 27 July and once again recommend that people in areas of high transmission wear masks indoors.
I want nothing more than for you to be right so we can get this show back on the road, but the only brightspot they offered regarding delta was:

However, vaccinated people with Delta might remain infectious for a shorter period, according to researchers in Singapore who tracked viral loads for each day of COVID-19 infection among people who had and hadn’t been vaccinated. Delta viral loads were similar for both groups for the first week of infection, but dropped quickly after day 7 in vaccinated people4.
From what I understand it's also very difficult to get data on vaccinated people given they're less likely to be tested but I don't know all the details of that. As I said, I really want for you to be right, and that the chances of me catching it or spreading it to someone who can't be vaccinated are exceedingly low, but I'm not brimming with optimism.

breakthrough infection
What is the next catchphrase do you think you'll be parroting?
religious zealots?
Is that about a year out?

LOL @ "breathrough infections"
Good lord man

The pandemic was over mr. doctor, as all are after 18 months and a couple waves.
Now it's a pandemic of sheer ignorance, and physically the poison injected into the world.
Have you taken a d-dimer test recently?

The guy that just claimed masks are communism is talking about people parroting things?

What about the Bubonic plague? Pretty sure that lasted a long ass time.

communism relates to means of production

the mask debate has nothing to do with means of production

and Biden is a corrupt corporatist to the bone
You're right about Biden, but:

Communism derives from the French communisme, which developed out of the Latin roots communis and the suffix isme. Semantically, communis can be translated to "of or for the community", while isme is a suffix that indicates the abstraction into a state, condition, action, or doctrine.
Mandatory masks do kind of fit into that "For the community" part.

touche

but lets be real. the republican politicians are throwing out the word communism left and right as a synonym for "democratic policies" despite the fact that Biden is bought out by the same people that they are. So he is aligned with them in that sense. He just differs from them (albeit slightly) in terms of healthcare, climate change, and social issues

The Bubonic plague was not a coronavirus.
And btw, "science" literature still teach that plague was carried by lice feces on rats, despite publushed studies nearly a decade ago proving that false.
Now that's scientific integrity eh?

Sure. If I need an answer to clinical presentation. Not to contact tracing.
Again, predictive value, pretest probability, etc.
But it doesn't give you an answer to clinical presentation.
It gives your administrators funding, that is the sole purpose of its use, as stated by its inventor.
In no way, shape or form does the test outline protocol.

But my question isn't to your patients presenting symptoms, it's to you specifically.
Are you aware of spike proteins causing microvascular coagulation not visible through other means eg ultrasound/cat, and only visible by d-dimer?
This with NO presentation?

Are you aware of the armed services drastic rise in concerning tests among the vaccinated?

Whoa, whoa, whoa, whoa, whoa, whoa, whoa. Sir, I was simply responding to your "All pandemics end in 18 months" claim. Nothing about coronavirus' or scientific integrity,
